Advocacy & Decision-Making Support
There are moments in life when paperwork feels like a different language, official letters make your heart sink, and decisions about care, health, or wellbeing feel overwhelming. That’s where we step in.
At Care Nav Support, we stand beside people who feel unheard or overlooked. Our team – made up of patient advocates, caring volunteers, and people with lived experience – offers more than just advice. We listen. We translate jargon into plain language. We sit with you at meetings, take notes, ask the questions you might not think of, and make sure your voice is not only heard but respected.
Whether it’s helping you navigate social services, speaking with healthcare professionals, or filling out complex forms, we’re here to remove the fear and confusion.
How we do it:
• One-to-one advocacy sessions at your home, our community hub, or over the phone.
• Liaising directly with professionals on your behalf.
• Guiding you through the decision-making process step-by-step, so you feel confident and in control.
We don’t make the decisions for you – we make sure you have the clarity and support to make them yourself.
Emotional & Mental Health Support
Sometimes what you need most isn’t a solution, it’s a safe space to say, “I’m not okay.”
We believe in the power of listening without judgement. Our emotional and mental health support is grounded in empathy, patience, and real human connection. Whether you’re going through a tough time, feeling isolated, or just need to talk something through, we’ll match you with someone who will hear you – really hear you.
Our trained listeners and peer mentors can check in regularly, help you spot when things are getting too heavy, and, if needed, gently guide you towards professional mental health services.
How we do it:
- Scheduled wellbeing check-ins by phone, video call, or face-to-face.
- Peer mentoring from volunteers who’ve faced similar struggles and found their way forward.
- Direct referrals to trusted local mental health services.
You don’t have to face the storm alone. We’ll be your steady anchor, however long you need us.
Cancer & Chronic Illness Support
A diagnosis can change everything in an instant. Suddenly, you’re juggling appointments, treatments, side effects, and emotions you didn’t know you could feel all at once. Families often feel just as lost and drained as the person who is unwell.
We offer gentle, consistent support through the journey – from diagnosis to recovery and beyond. That could mean sitting quietly with you during treatment, making a cup of tea and chatting on a low-energy day, or helping you and your loved ones find practical resources to make life a little easier.
How we do it:
- Emotional check-ins for both patients and families.
- Guidance through treatment plans, paperwork, and service options.
- Companionship visits to ease loneliness and lift spirits.
We know we can’t take away the illness – but we can make sure you never feel alone in facing it.
Elderly Support & Companionship
Growing older should never mean growing invisible. Too many older people spend days without meaningful conversation or the reassurance of a friendly face.
Our Elderly Support & Companionship service is about bringing warmth back into the everyday. We pair older adults with compassionate volunteers who drop by for tea, help with small errands, share memories, and simply spend time together.
We also help with the practical side of life – making sure important letters are understood, bills are sorted, appointments are attended, and services are accessed.
How we do it:
- Friendly home visits and regular phone calls to beat loneliness.
- Help with navigating local services and entitlements.
- Day-to-day advocacy for issues that can otherwise pile up.
Every visit is more than a duty – it’s a chance to brighten someone’s day and remind them they still matter deeply to their community.
Disability Support
Living with a disability – or caring for someone who does – often means facing barriers that most people never notice. From inaccessible services to confusing benefits systems, the challenges can feel endless.
We’re here to break down those barriers. Our support focuses on rights, resources, and resilience – helping people with disabilities and their families navigate the world with confidence.
How we do it:
- Personal guidance on rights and entitlements.
- Emotional encouragement and peer mentoring from those who truly understand.
- Support for families to make caring less stressful and more sustainable.
We can’t promise to fix every problem, but we promise to stand beside you while you face them.
Mental Health Navigation
When mental health struggles hit, knowing where to turn can feel impossible. Crisis can make everything blurry – decisions harder, tasks heavier, and hope harder to find.
We act as a guiding hand in that moment. Our volunteers offer immediate emotional grounding, help you figure out the next step, and make sure you’re connected to the right services quickly.
How we do it:
- Crisis listening by phone, online chat, or in person.
- Emotional grounding techniques to help you regain stability.
- Follow-up to make sure you’ve reached the right kind of help.
You don’t have to navigate the maze alone. We’ll walk with you until you’re back on steadier ground.
Family & Carer Support
Caring for a loved one can be one of the most meaningful acts of love – but also one of the most exhausting. Many carers carry the weight of two lives, often in silence, until they burn out.
We give carers a safe place to lay down that weight for a while. Whether it’s a coffee with someone who truly understands, access to community resources, or just an hour of uninterrupted rest, we make sure carers are cared for too.
How we do it:
- Small, welcoming peer support groups where carers can talk freely.
- One-to-one emotional check-ins to make sure you’re coping.
- Connections to respite services and practical help.
Caring should never mean sacrificing your own health or happiness – and we’re here to help you keep that balance.
Addiction Recovery Guidance
Recovery is not a straight line. It’s a series of steps – sometimes forward, sometimes back – and it’s much easier to walk that path with someone who has walked it before.
Our Addiction Recovery Guidance pairs people in recovery with volunteers who have lived through it themselves. They understand the setbacks, the victories, and the quiet daily work of staying well.
How we do it:
- Non-judgemental one-to-one support.
- Weekly check-ins for encouragement and accountability.
- Guidance to connect with recovery programs, therapy, and community services.
We believe in second chances – and third, and fourth – because recovery is always worth fighting for.
